HNL Lab Medicine offers collection services in more than 50 patient service centers in Pennsylvania. Testing samples are collected in patient service center locations identified on the HNL Lab Medicine Website. Testing samples are forwarded to a licensed clinical laboratory which maintains a current CLIA (Clinical Laboratory Improvement Amendments) certificate and appropriate permits and accreditations.

Choice of Law and Venue

Cancellation/Refund
Cancellation of a test or the entire request prior to collection of testing samples will result in an applicable refund, less a 10 percent processing fee. Refunds will not be made after testing samples have been collected except in limited circumstances when HNL Lab Tests Direct is unable to process your sample. In these circumstances, you will receive a full (100%) refund for the test(s) not processed.
Your laboratory requisition form expires ninety (180) days from the date of the request. Refunds will not be made after the expiration date of the requisition form.
Restrictions
You are specifically restricted from all of the following: publishing any Website material in any other media; selling, sublicensing and/or otherwise commercializing any Website material; publicly performing and/or showing any Website material; using this Website in any way that is or may be damaging to this Website; using this Website in any way that impacts user access to this Website; using this Website contrary to applicable laws and regulations, or in any way may cause harm to the Website, or to any person or business entity; engaging in any data mining, data harvesting, data extracting or any other similar activity in relation to this Website; using this Website to engage in any advertising or marketing.
Certain areas of this Website are restricted from being access by you and HNL Lab Medicine may further restrict access by you to any areas of this Website, at any time, in absolute discretion. Any user ID and password you may have for this Website are confidential and you must maintain confidentiality as well.
Minors and Dependent Accounts

Discount/Coupon Policy
Discount coupon codes issued by HNL Lab Medicine are issued as a courtesy to our customers. Orders placed that do not meet the requirements or after expiration will be processed without the discount. Monthly specials will only be honored during the month they are featured. Refunds will not be given retroactively. Coupons are not redeemable for cash and only one may be used for each purchase. Adjustments, credits and/or refunds for discount codes not entered at the time of ordering will not be issued.
HNL Lab Tests Direct Reward Terms of Use

Insurance Claims
As an online service, HNL Lab Medicine’s direct-to-consumer service, LTD, does not accept medical insurance, nor do we provide services, information, or assistance for submitting claims to insurance companies. These services may or may not be covered by your Health Insurance company.
Pricing
HNL Lab Medicine prices are only available for tests that are ordered and purchased through HNL Lab Medicine’s direct-to-consumer service, LTD, service.
Marketing
By registering an account with us, you agree to receive email marketing that provides promotions and health information. At any time, you may opt out of these emails as indicated on the email.
SHARED ACCOUNT ACKNOWLEDGMENT

In addition to shared accounts, patients may elect to share their laboratory results with their healthcare provider. The patient must provide explicit consent in order to share their health information with a healthcare provider external to HNL Lab Medicine and their affiliates.
Responsibilities
The patient is responsible for managing and granting access to their health information through the shared account. Shared account users must use the website in accordance with these terms and respect the confidentiality of the patient's health information.
Access Permissions
The patient has control over the level of access granted to shared account users, as well as the information they share with healthcare providers. The patient can revoke shared account access at any time without prior notice.
Security
Shared account users must maintain the confidentiality of their login credentials. In the event of unauthorized access, the patient must promptly notify HNL Lab Medicine.
Use of Information
Shared account users may only access the patient's health information for authorized purposes related to the patient's healthcare. Similarly, healthcare providers outside of HNL Lab Medicine and our affiliates may only access the patient's health information that the patient elects to share with them. Users are prohibited from sharing account information, attempting to access unauthorized information, or using the Patient Portal for any illegal or unethical activities.
Termination
The patient can terminate shared account access at any time. HNL Lab Medicine reserves the right to terminate shared account access in case of a violation of these terms.

These Terms of Use are entered into in the Commonwealth of Pennsylvania, U.S.A. Each party to these Terms of Use submits to the exclusive jurisdiction of the state and federal courts sitting in Lehigh County in the Commonwealth of Pennsylvania, U.S.A., and waives any jurisdictional, venue, or inconvenient forum objections to such courts. In any action to enforce these Terms of Use, the prevailing party will be entitled to costs and attorneys’ fees. In the event that any of the Terms of Use are held by a court or other tribunal of competent jurisdiction to be unenforceable, such provisions shall be limited or eliminated to the minimum extent necessary so that these Terms of Use shall otherwise remain in full force and effect.
Cancellation/Refund
Cancellation of a test or the entire request prior to collection of testing samples will result in an applicable refund, less a 10 percent processing fee. Refunds will not be made after testing samples have been collected except in limited circumstances when HNL Lab Tests Direct is unable to process your sample. In these circumstances, you will receive a full (100%) refund for the test(s) not processed.
Your laboratory requisition form expires ninety (180) days from the date of the request. Refunds will not be made after the expiration date of the requisition form.
Restrictions
You are specifically restricted from all of the following: publishing any Website material in any other media; selling, sublicensing and/or otherwise commercializing any Website material; publicly performing and/or showing any Website material; using this Website in any way that is or may be damaging to this Website; using this Website in any way that impacts user access to this Website; using this Website contrary to applicable laws and regulations, or in any way may cause harm to the Website, or to any person or business entity; engaging in any data mining, data harvesting, data extracting or any other similar activity in relation to this Website; using this Website to engage in any advertising or marketing.
Certain areas of this Website are restricted from being access by you and HNL Lab Medicine may further restrict access by you to any areas of this Website, at any time, in absolute discretion. Any user ID and password you may have for this Website are confidential and you must maintain confidentiality as well.
Minors and Dependent Accounts

Discount/Coupon Policy
Discount coupon codes issued by HNL Lab Medicine are issued as a courtesy to our customers. Orders placed that do not meet the requirements or after expiration will be processed without the discount. Monthly specials will only be honored during the month they are featured. Refunds will not be given retroactively. Coupons are not redeemable for cash and only one may be used for each purchase. Adjustments, credits and/or refunds for discount codes not entered at the time of ordering will not be issued.

Insurance Claims
As an online service, HNL Lab Medicine’s direct-to-consumer service, LTD, does not accept medical insurance, nor do we provide services, information, or assistance for submitting claims to insurance companies. These services may or may not be covered by your Health Insurance company.

HNL Lab Medicine prices are only available for tests that are ordered and purchased through HNL Lab Medicine’s direct-to-consumer service, LTD, service.

In addition to shared accounts, patients may elect to share their laboratory results with their healthcare provider. The patient must provide explicit consent in order to share their health information with a healthcare provider external to HNL Lab Medicine and their affiliates.
Responsibilities
The patient is responsible for managing and granting access to their health information through the shared account. Shared account users must use the website in accordance with these terms and respect the confidentiality of the patient's health information.
Access Permissions
The patient has control over the level of access granted to shared account users, as well as the information they share with healthcare providers. The patient can revoke shared account access at any time without prior notice.
Security
Shared account users must maintain the confidentiality of their login credentials. In the event of unauthorized access, the patient must promptly notify HNL Lab Medicine.
Use of Information
Shared account users may only access the patient's health information for authorized purposes related to the patient's healthcare. Similarly, healthcare providers outside of HNL Lab Medicine and our affiliates may only access the patient's health information that the patient elects to share with them. Users are prohibited from sharing account information, attempting to access unauthorized information, or using the Patient Portal for any illegal or unethical activities.
Termination
The patient can terminate shared account access at any time. HNL Lab Medicine reserves the right to terminate shared account access in case of a violation of these terms.
Updates to Terms
Users will be notified of any updates to these terms. Continued use of the shared account implies acceptance of the updated terms.
Limitation of Liability
HNL Lab Medicine is not liable for any damages arising from the use or inability to use this website, including but not limited to unauthorized access.
Governing Law
